Submit a Referral
Complete our referral form and submit the requested supporting documentation to Divine Intensive Inc.

Referral Review
Our team reviews the referral to learn about the individual’s needs, preferences, waiver services, and current support needs.

Initial Contact
We contact the individual, family/guardian, and/or Support Coordinator to discuss the referral, answer questions, and determine whether Divine Intensive may be an appropriate provider.

Meet & Discuss Services
When appropriate, we arrange an introduction or meeting to discuss the requested service, individual preferences, staffing needs, and expectations.

Service Coordination & Start
If Divine Intensive is selected and able to meet the individual’s needs, we work with the Support Coordinator and support team to complete the necessary admission and service-start requirements.
Who Is Eligible?
Divine Intensive Inc. provides services to individuals with developmental and/or intellectual disabilities who need support to live safely, independently, and meaningfully in their homes and communities.
- Individuals may be eligible for services if they:
• Have a developmental or intellectual disability.
• Are enrolled in or eligible for Virginia’s FIS, CL, or BI Medicaid Waiver, depending on the service requested.
• Have an identified need for one or more services offered by Divine Intensive.
• Have a Support Coordinator or appropriate referral source assisting with service coordination.
• Choose Divine Intensive Inc. as their provider and complete the required referral and admission process.
